Ava Connect Patient Portal

1. We need your email address. Call us and give us your email address, and we will send you an invite to join Ava Connect. This invite only lasts for 4 days, after which it expires, so please sign up promptly. Please note, the portal will only show information pertaining to you and your medical information alone. For additional family members, you will need to provide separate email addresses, and sign up for each member separately.

2. Open the email invite (check junk folder if you don't see it in the inbox) and follow the link to avaconnect.ca. For more detailed instructions, click here.

3. Complete the sign up by providing your email, birthday, password, etc, as prompted.

4. Read and accept the Terms of Service.

5. Complete the registration by confirming your email with the confirmation code you were sent. It may take a few minutes to show up in your inbox.

That's it! Now you can access the portal any time by going to avaconnect.ca and signing in. If you forgot your password you can always reset it.
