I am writing to inform you that my family medicine practice will be closing.

After careful consideration, I will be changing my scope of practice to provide care in urgent care settings and to expand my women’s health and menopause clinic at a different location.

I will also continue my migraine Botox clinic. Patients currently receiving Botox treatments will be contacted with information about continuing their care at my new office.

It has been a privilege to care for you over the past 15 years. I sincerely thank you for your trust, and I wish you all the very best going forward.

This notice is being provided more than 90 days in advance to support your transition to a new healthcare provider.

I strongly encourage you to begin looking for a new family physician as soon as possible.

• Ask your specialist(s) if they can assist with interim care

Dr. Arjang and Dr. Fatoorechi will continue their practices at new locations and will communicate directly with their patients. Dr. Fatoorechi may have limited availability for new patients; you may contact this office to inquire about a meet-and-greet appointment.

If you have ongoing or complex medical needs, I strongly recommend booking an appointment before the clinic closes to:

• Review your current health concerns
• Update your medication list
• Clarify follow-up plans
• Ensure your medical record is up to date for your next provider

Until my final clinical day, I will continue to provide medically necessary care.

As the closure date approaches, priority may be given to:

• Urgent medical concerns
• Transition planning

I may not be able to complete:

• New non-urgent referrals or imaging
• Insurance or third-party forms
• Long-term or complex paperwork

• Please ensure your medications are reviewed before closure
• I will provide reasonable prescription renewals to support continuity of care (short-term bridging only)
• Review any outstanding test results prior to closure

If you have:

• Standing orders under my name → discuss these with your new provider
• Booked imaging (e.g., MRI, CT) or specialist visits → these may proceed as planned

If you are receiving controlled medications and are having difficulty finding a new prescriber, you may contact:

After the clinic closes:

• PCN services like primary care nursing - Margot and psychology services - Julie,  at this clinic will no longer be available, so please conclude your treatment goals with them

Ideally your records should be transferred by fax to your new Family doctor or primary care provider. Please let your new office send us a signed request for chart transfer and the reception staff will forward your information to them.

Your medical records will also be securely retained in accordance with Alberta’s Health Information Act.

After clinic closure:

• Information about how to request your records will be available through the
  College of Physicians and Surgeons of Alberta website https://CPSA.ca
• A reasonable administrative fee may apply, as permitted by legislation

Important:

• Clinic phone, email, and patient portal access will not be available after closure
• Please download or save any needed information before August 31, 2026

Most key health information (lab results, imaging, specialist reports) is available to healthcare providers via:

You may also access parts of your health information through:

If you are unable to secure a new family physician before my final day, I will work with you to:

• Provide short-term medication coverage
• Outline interim care plans
• Help ensure safe transition of care

I recognize that changes in medical care can be stressful, particularly for patients with ongoing or complex health needs.
